For your convenience, you can download our patient forms. Please bring these completed forms to your appointment.

Insurance Information

We accept most dental insurance plans. As long as you have the freedom to choose your provider (PPO) we should accept your plan. We do not accept HMO or DMO insurance plans.

Each plan is different and the benefits may be difficult to understand. If you are unsure of your benefits please call and Sandi or Melinda would be happy to clarify your patient portion.

Payment Options

We accept cash, check, credit or debit card.

For your convenience, we are pleased to accept Care Credit. Call to learn more about how you can break up your payments with NO INTEREST.
